About Reaching Abilities and Our NDIS Services

To be eligible for the NDIS, you must:

• Have a permanent and significant disability that affects your ability to take part in everyday activities.
• Be aged under 65 when you first apply to join the NDIS.
• Be an Australian citizen, a permanent resident, or hold a Protected Special Category Visa.

If you meet these criteria, you can apply for NDIS support. Reaching Abilities can help guide you through the application process.

To apply for NDIS funding, follow these steps:

1. Check your eligibility on the NDIS website.
2. Gather necessary documents, including proof of disability and identity.
3. Submit an Access Request Form to the NDIS.
4. Once approved, you will work with a planner to develop your NDIS plan.

The NDIS provides funding for a variety of supports, including:

• Daily personal activities
• Transport to enable participation in community, social, economic, and daily life activities
• Workplace help to allow a participant to successfully get or keep employment in the open or supported labor market
• Therapeutic supports, including behavior support
• Help with household tasks to allow the participant to maintain their home environment
• Skilled personnel in aids or equipment assessment, set up, and training

NDIS plans can be managed in three ways:

1. Self-managed: Participants manage their funding and supports.
2. Plan-managed: A Plan Manager is funded in your plan to manage your funding for you.
3. NDIA-managed: The National Disability Insurance Agency (NDIA) manages your funding and pays providers directly.

Reaching Abilities provides both plan management and support coordination services to help you manage your NDIS plan effectively.

During your NDIS planning meeting, you will:

• Discuss your current supports and any additional support you may need.
• Set goals you want to achieve with the help of NDIS funding.
• Identify the types of support you will need to meet these goals.
